Intriguingly, the best results were obtained under the assumption that Hh signaling cannot alter the overall tension force of the cell, but will merely re-distribute it locally inside the cell, relative to the signaling status of neighboring cells. “To review our Pediatric Endocrinology Division’s experience with differentiated thyroid carcinoma (DTC) we analyzed retrospectively the records of patients with DTC that had been seen between June 1988 and June 2008.

Results: Forty-five patients (median age 13.7 years, 36 female) were diagnosed (papillary: 40, follicular: 5) with DTC presenting as a solitary nodule (n: 25), thyroid nodule with cervical adenopathy (n: 9) and multinodular goiter (n: 11). All underwent total thyroidectomy with resection of suspicious cervical lymph nodes (CLN). DTC was multicentric in 59% and revealed extrathyroidal extension in 44%. Initially, 44% had CLN metastases and 24% distant metastases. All patients underwent thyroid remnant ablation with (131)I and suppressive treatment. Median follow-up was 5.1 years with a disease-free survival rate at 5 years of follow-up of 75%. Eleven percent presented recurrences.

OBJECTIVE: To assess the effectiveness of highly active antiretroviral therapy (HAART) in reducing incident tuberculosis (TB) in HIV-infected

children.

DESIGN: Retrospective cohort.

RESULTS: A total of 1132 children’s records were included in the study. At R428 cell line entry to the cohort,

the median (interquartile range [IQR]) age, CD4%, CD4 count and viral load of all children was respectively 6.3 years (4.1-8.8), 15% (9.0-22.2), 576 cells/mm(3) (287-960) and 160000 copies/ml (54941.5-449683); 75.9% were started on HAART. The male:female ratio was 1:1, and median follow-up time was 1.7 years. In children whose follow-up included both pre-HAART and on-HAART periods, the incidence of clinically diagnosed TB was respectively 21.1 per 100 person-years (py; 95% CI 18.2-24.4) and 6.4/100 py (95% CI 4.8-8.1), and when restricted to confirmed cases, respectively 3.1/100 py (95% CI 2.2-4.2) and 0.8/1.00 py (95% CI 0.5-1.4). Only 23% of all cases of TB were microbiologically confirmed. Multivariate analyses showed that HAART reduced incident TB by approximately 70%, both for confirmed and all TB cases.

CONCLUSIONS: In this high TB burden country, the incidence of diagnosis of TB in HIV-infected children is at least as high as that of adults. HAART reduces incident TB, but further prospective TB preventive and diagnostic studies are urgently needed in children.”

Comparative studies on dietary patterns and long-term mortality are sparse.

Objective: The objective was to examine the relations between 10-y mortality and adherence to the World Health Organization dietary guidelines [Healthy Diet Indicator (HDI)], a Mediterranean-like diet, and a carbohydrate-restricted (CR) diet in elderly Swedish men.

Design: Dietary habits were determined by 7-d dietary records in a population-based this website longitudinal study of 924 Swedish men (age: 71 +/- 1 y). The HDI score (-1 to 8 points), the Mediterranean Diet Score (MDS; 0-8 points), and the CR score (2-20 points) were calculated for each participant. “Human embryo vitrification is a promising new technology but clinical outcome data is needed to gauge its effectiveness and safety. While pregnancy and live-birth data is available for blastocyst vitrification, such information is lacking for human embryo vitrification at the 6- to 8-cell

stage. The current work presents clinical and obstetric outcomes from the transfer of embryos vitrified on day 3 at the cleavage stage. A total of 270 transfers were performed. The clinical pregnancy and implantation rates for patients under 38 years of age (n = 200) were 45% and 24%, respectively. Corresponding rates in patients 38-42 years old declined to 29% and 13% (n = 70). Embryonic compaction and/or blastulation by the time of transfer were excellent prognostic indicators of a successful pregnancy outcome.
