The phoD gene, encoding alkaline phosphatase (ALP) in bacteria, contributes importantly to the release of soluble reactive phosphorus (SRP) from organic phosphorus within ecosystems. However, the gene phoD's diversity and prevalence in ecosystems are insufficiently characterized. To study Sancha Lake, a typical eutrophic sub-deep freshwater lake in China, nine different sampling locations were selected. Surface sediment and overlying water were taken on April 15th (spring) and November 3rd (autumn), 2017. To determine the diversity and abundance of the bacterial phoD gene in sediments, high-throughput sequencing and qPCR methods were utilized. A further examination of the correlation between phoD gene diversity and abundance, environmental influences, and ALP activity was undertaken. Out of 18 samples, 881,717 valid sequences were extracted and categorized into 477 OTUs, further comprising 41 genera, 31 families, 23 orders, 12 classes, and 9 phyla. The most significant phyla, Proteobacteria and Actinobacteria, were prominent. Three branches formed the phylogenetic tree diagrammed based on the phoD gene sequences. A substantial proportion of the aligned genetic sequences corresponded to the genera Pseudomonas, Streptomyces, Cupriavidus, and Paludisphaer. Spring and autumn bacterial communities, enriched for phoD, exhibited a substantial structural difference, with no noticeable spatial heterogeneity. Preoperative discussions at a multidisciplinary conference concerning high-risk operative spine patients, may potentially minimize adverse outcomes via patient selection refinement and surgical strategy refinement. To attain this desired outcome, a high-risk case conference was conducted incorporating specialists from orthopedics and neurosurgery spine, anesthesia, intraoperative monitoring neurology, and neurological intensive care departments.
This retrospective study examined patients aged 18 and over who presented with one or more of the following high-risk factors: fusion of eight or more spinal levels, osteoporosis associated with four or more fused levels, three-column osteotomy, revision of the anterior portion of the same lumbar segment, or a planned substantial correction for severe myelopathy, scoliosis exceeding 75 degrees, or kyphosis exceeding 75 degrees. Patients undergoing surgery before February 19, 2019, were designated as Before Conference (BC), contrasting with After Conference (AC) surgery for patients who underwent their procedure thereafter. A cohort of 263 patients was selected for this study; 96 patients belonged to the AC arm and 167 to the BC arm. Group AC was significantly older (600 years versus 546 years, p=0.0025) and had a lower BMI (271 vs 289, p=0.0047) than group BC. However, CCI (32 vs 29, p=0.0312) and ASA classification (25 vs 25, p=0.790) were similar. The surgical interventions in AC and BC groups demonstrated similar profiles, with respect to the fusion levels (106 vs 107, p=0.839), decompressed levels (129 vs 125, p=0.863), three column osteotomies (104% vs 186%, p=0.0080), anterior column release (94% vs 126%, p=0.432), and revision cases (531% vs 524%, p=0.911). While AC demonstrated a lower prevalence of deep surgical site infections (10% SSI) compared to the control group (66%), p=0.0038, a substantially higher proportion of AC patients experienced hypotension requiring vasopressor treatment (188% vs 48%), p<0.0001. The spectrum of postoperative complications remained consistent amongst the two groups. AC procedures exhibited a substantial decrease in reoperation rates at both 30 (21% versus 84%, p=0.0040) and 90 days (31% versus 120%, p=0.0014). Furthermore, significantly reduced readmission rates were also observed: 31% at 30 days (versus 102%, p=0.0038) and 63% at 90 days (versus 150%, p=0.0035) after the AC procedure.
